The Florida Department of Health in Escambia County is thrilled to announce the launch of a new program called the Chronic Disease Self-Management Program (CDSMP).

This program is designed for adults who have at least one chronic health condition and aims to provide them with the tools and knowledge they need to manage their condition more effectively. The CDSMP is an interactive workshop that covers several areas of chronic disease management, such as symptom management, healthy eating, physical activity, communication, medication management, and fatigue management. The program’s primary objective is to empower participants to take control of their health and well-being and live fuller, healthier lives.

The program is set to begin on Tuesday, January 23, 2024, and will be conducted over 6 weeks, with each session lasting around two and a half hours. Participants will be encouraged to attend all sessions to get the maximum benefit from the program.

The CDSMP is open to all adults with at least one chronic health condition who are interested in improving their health and well-being. Enrollment is easy!
